Brief Description of Duties: This position is located in the Department of Health (DOH), Community Health Administration (CHA), Child, Adolescent and School Health Bureau (CASH), School Health Division (SHD), Immunization Program. The Immunization Program is responsible for improving immunization protection levels and for controlling the existence or spread of disease and reducing the morbidity and mortality rate due to vaccine preventable diseases of childhood. The incumbent serves as an investigator and is responsible for performing medical chart reviews at public and private provider offices in order to determine provider immunization coverage levels. Works with physicians to develop a plan to increase immunization coverage level in their practice. Performs physician visitations in accordance with current policy and as directed by the supervisor to ensure providers compliance with Federal guidelines. Documents all aspects of work and prepares and maintains accurate and complete records and reports.
